Registered Dietitian

RD Nutrition Consultants LLC is seeking a Registered Dietitian for a flexible parttime opportunity at two skilled nursing facilities in Indianapolis.

Position Highlights:

• Flexible Schedule: 24hour weekly schedule divided between two conveniently located skilled nursing facilities (16 hours at one 8 hours at the other)

Responsibilities:

• Conduct comprehensive nutritional assessments of residents including diet history anthropometric measurements laboratory data review and clinical evaluation.
  
• Develop and implement individualized nutrition care plans to address identified nutritional needs and goals.
  
• Provide nutrition education and counseling to residents and their families tailored to their specific needs and understanding levels.
  
• Monitor residents nutritional status and progress making adjustments to care plans as needed.
  
• Collaborate with the interdisciplinary care team including physicians nurses therapists and social workers to ensure comprehensive and coordinated care.
  
• Maintain accurate and complete medical records including nutrition assessments care plans progress notes and discharge summaries.
  

Requirements:

• Registered Dietitian with active registration with the Commission on Dietetic Registration.
  
• Strong clinical assessment and care planning skills.
  
• Excellent communication and interpersonal skills with the ability to effectively educate and counsel residents and families.
  
• Proficiency with electronic documentation and menu systems.
  
• Ability to maintain confidentiality of patient information.
  
• Flexibility and willingness to travel between two facility locations.
