Job Description

- Ensures the referral processes are running smoothly according to the referral’s guidelines.
- Manages incoming and outgoing referral processes
- Provides the required updates for the referral policies and pathway whenever needed
- Provide the required training for the Focal Points, Field Monitors and any other related staff.
- Supervise the Hospitals Focal Points and Field Monitors.
- He/she is responsible for the overall management of referral focal points at hospitals level
- Monitors and ensures the proper application of the Referral Policies by following the referral processes and theirs outcomes
- Conducts regular field visits, whenever required, to ensure successful application of referral system
- Builds and maintain good relationships with relevant agencies and service providers to support the referral pathways
