KeystoneCare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide private duty nursing care for a patient with ALS in their home. This role is ideal for a nurse who is committed to delivering high-quality patient-centered care while supporting both the patient and their family.

Primary Job Responsibilities:
- Provide skilled nursing care in accordance with physician orders patient care plans and agency policies
- Administer medications and treatments as prescribed
- Perform ongoing patient assessments and monitor for changes in condition
- Ensure patient safety comfort and overall well-being throughout the shift
- Communicate effectively with patients families and interdisciplinary care teams
- Educate patients and families on care plans equipment use and safety measures
- Maintain accurate and timely documentation in compliance with agency standards
Requirements
Job Requirements:
- Graduate of an accredited LPN program
- Current LPN license in Pennsylvania
- Minimum of one year experience in home healthcare or a similar clinical setting
- Familiarity with Feeding Tubes Assisted breathing Devices (Ventilators) and Hoyer Lift
- CPR certification
- Strong communication and interpersonal skills
- Self-motivated with the ability to work independently
- Reliable transportation and valid Drivers License
