Hospice Director of Health Care Services (DHCS) & RN Manager

Position Summary

TheHospice Director of Health Care Services (DHCS) & RN Manageris responsible forthe overall clinical oversight coordination and delivery of hospice care services across all settings. This role integrates leadership responsibilities with direct patient care functions including case management on-call support and start-of-care (SOC) services ensuring high-quality compliant patient-centered DHCS & RN Manager ensures that care delivery aligns with hospice regulations organizational policies and the interdisciplinary plan of care while supporting staff development operational efficiency and quality outcomes.

Essential Leadership & Operational Functions (DHCS Role)

  • Oversees the planning coordination and evaluation of hospice clinical services to ensure compliance with regulatory requirements standards of practice and organizational policies.

  • Ensuresappropriate staffinglevels including recruitment onboarding education supervision and retention of qualified hospice personnel.

  • Provides clinical leadership and supervision to nursing staff aides and interdisciplinary team members.

  • Ensures ongoing staff competency education and adherence to hospice philosophy regulations and QAPI initiatives.

  • Oversees patient care quality documentation and compliance with federal/state regulations and accreditation standards.

  • Participates in budget planning operational strategy and performance improvement initiatives.

  • Communicates regularly with theDirector of Operations/Clinical Operationsregardingclinical operations quality metrics incidents and regulatory compliance.

  • Maintains oversight of patient records reports and clinical data to support compliance and performance monitoring.

Clinical Case Management Responsibilities (RN Manager Role)

  • Completes comprehensive patient assessments including physical psychosocial and symptom management needs.

  • Develops implements and revises individualized plans of care in collaboration with the interdisciplinary group (IDG) patient and physician.

  • Coordinates care across all disciplines to ensure continuity and achievement of hospice goals.

  • Monitors patient condition and adjusts care plans based on changing needs.

  • Provides patient and family education related to disease progression symptom management and care plan.

  • Administers medications and treatments perphysicianorders and standards of practice.

  • Supervises LPNs/LVNs hospice aides and other assigned staff.

  • Participates in IDG meetings and contributes to care planning and outcomes.

  • Maintainsaccurateandtimelyclinical documentation.

On-Call SOC and After-Hours Responsibilities

  • Participates in and may oversee on-call scheduling and coverage to ensure 24/7 patient support.

  • Performs on-call duties including triage symptom management visits and crisis intervention.

  • Completes Start of Care (SOC) visits death visits and urgent patient assessments as needed.

  • Provides follow-up visits and after-hours support for patients with acute needs or transitions in care.

  • Coordinates with answering services and interdisciplinary team members to ensure seamless care delivery.

Quality Compliance & Performance

  • Ensures adherence to CMS Conditions of Participation state regulations and accreditation standards.

  • Leads andparticipatesin QAPI activities including audits performance improvement initiatives and outcome tracking.

  • Monitors clinical documentation for accuracy timeliness and regulatory compliance.

  • Promotes a culture of safety patient-centered care and continuous improvement.

MinimumEducation & Experience Requirements

  • Graduate of an accredited school of nursing; BSN preferred.

  • Active unrestricted RN license in the state of practice.

  • Minimum of3years hospice nursing experience preferred.

  • Minimum of1-2years of healthcare management or supervisory experience preferred.

  • Demonstrated leadership experience in hospice or related healthcaresetting.

Knowledge Skills & Abilities

  • Strong knowledge of hospice regulations clinical standards and interdisciplinary care models.

  • Expertise inpain and symptom management.

  • Effective leadership team development and staff supervision skills.

  • Excellent communication problem-solving and organizational abilities.

  • Ability to manage multiple priorities in a dynamic clinical environment.

  • Commitment to hospice philosophy and patient-centered care.

Physical & Work Environment

  • Combination of office-based work and field visits in patient homes hospitals and facilities.

  • Frequent travel withinservicearea.

  • Exposure to clinical environments and infectious materials requiringappropriate precautions.

  • Able to constantly stand walk bend stoop squat kneel and reach freely andassistpatients with standing walking and transferring.

  • Sufficient endurance to perform the required physical and mental tasks overlong periodsof time.

  • Able to constantly grasp with thumb in opposition to fingers of penknobsor objects.

  • Visual acuitydexterityand hearing to perform required nursing skills.

  • Able tofrequentlywork on-call hours and evening night and weekend hours as needed.

  • Medium physical activity that may require frequent lifting carryingpushingor pulling up to out daily job functions and related activities that may berequired.

  • Maintains CPR certification valid drivers license/insurance and required screenings.
