Contract Duration: 10 Weeks
Shift: Day/Night Rotation (5:45 AM 6:15 PM / 5:45 PM 6:15 AM)
Guaranteed Hours: 36 hours/week

Job Summary

Navitas Healthcare LLC is seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a short-term 10-week contract assignment in a skilled nursing/long-term care setting. The ideal candidate will provide high-quality patient care support daily nursing operations and collaborate with the interdisciplinary care team to ensure patient safety comfort and well-being.

Key Responsibilities

• Provide direct nursing care under the supervision of an RN or physician
• Administer medications and treatments as prescribed
• Monitor document and report patient conditions vital signs and treatment responses
• Assist patients with Activities of Daily Living (ADLs) including hygiene mobility and comfort care
• Maintain accurate and timely clinical documentation per facility standards
• Communicate effectively with healthcare team members patients and families regarding care plans and updates
• Observe and promptly report changes in patient condition
• Follow all infection control safety and regulatory compliance procedures
• Maintain a clean safe and organized patient care environment
• Support additional clinical and nursing duties as assigned

Qualifications

• Active and unrestricted LPN License required
• Current BLS/CPR certification preferred
• Prior experience in long-term care rehabilitation or skilled nursing strongly preferred
• Strong clinical assessment and patient care skills
• Excellent communication and teamwork abilities
• Ability to perform effectively in a fast-paced healthcare environment
• Compassionate reliable and patient-focused approach to care
