We are seeking multiple dedicated Licensed Vocational Nurse (LVN) to join our compassionate Home Health & Hospice team in Brentwood. Responsibilities
- Provide direct patient care and support under the supervision of registered nurses.
- Administer medications and monitor patient responses.
- Assist patients with daily living activities and personal hygiene.
- Document patient care and progress in medical records.
- Collaborate with healthcare team members to develop and implement care plans.
- Educate patients and families on health management and care procedures.
- Ensure compliance with health and safety regulations.
- Participate in ongoing training and professional development.
Requirements
- Completion of an accredited LVN program.
- Valid LVN license in the state of California.
- Strong clinical skills and knowledge of patient care practices.
