Job Summary
Human Touch Home Health is hiring a PRN Physical Therapist to serve patients throughout Fairfax Loudoun and Prince William Counties Virginia.
This role is ideal for a Physical Therapist who wants flexibility independence and the ability to provide one-on-one care in the home setting.
What Youll Do
- Perform physical therapy evaluations and reassessments
- Develop and implement individualized treatment plans
- Provide skilled therapy services in patients homes
- Coordinate care with physicians and the clinical team
- Complete documentation accurately and on time

Requirements
- Active Virginia Physical Therapist license
- Current CPR certification preferred
- Valid drivers license and reliable transportation
- Strong communication and documentation skills
- Home health experience preferred but not required
