Job Summary
Join The Home with a Heart and make a difference every day by supporting our residents health appointments transportation and overall well-being.
What Youll Do:
- Coordinate medical dental therapy and personal appointments
- Schedule and arrange resident transportation
- Escort residents safely to and from appointments
- Work closely with residents families care teams and providers
- Supervise and assist the Transportation Aide
- Maintain appointment and transportation records
- Provide compassionate resident-centered support
Qualifications:
- Current Pennsylvania CNA certification required
- Valid drivers license with clean driving record
- Strong communication and organizational skills
- Healthcare or long-term care experience preferred
- Comfortable with scheduling/documentation systems
Physical Requirements:
- Ability to lift and transfer up to 50 lbs.
- Frequent walking standing and wheelchair assistance
- Ability to assist residents in and out of vehicles
