Pay Range: $29.00-$39.00 per hour
Schedule: M-F 8:00-4:30pm or 8:30-5pm

What You Bring (Requirements)
- Bachelor Degree in Social Work
- Strong communication and customer service skills
- Ability to follow care plans and resident orders
- Long-term care experience (preferred)
What Youll Be Responsible For
- Support residents physical and emotional well-being by building meaningful connections and delivering compassionate person-centered care.
- Maintain a clean safe and comfortable environment while following all care plans safety protocols and infection control standards.
- Leads and directs Social Service Department.
- Coordinates resident admissions.
- Assist with prompt referral to an appropriate agency or government program if financial assistance is indicated; assist with re-certification papers to maintain status of government programs such as Medical Assistance Social Security and S.S.I.; notify Business Office.
- Completes assessments of residents as necessary for care planning MDS RAPS PPS and other required documents.
- Serves as resident advocate coordinates invitation of residents and families.
