Pay Range: $29.00-$39.00 per hour
Schedule: M-F 8:00-4:30pm or 8:30-5pm

What You Bring (Requirements)

• Bachelor Degree in Social Work
• Strong communication and customer service skills
• Ability to follow care plans and resident orders
• Long-term care experience (preferred)

What Youll Be Responsible For

• Support residents physical and emotional well-being by building meaningful connections and delivering compassionate person-centered care.
• Maintain a clean safe and comfortable environment while following all care plans safety protocols and infection control standards.
• Leads and directs Social Service Department.
• Coordinates resident admissions.
• Assist with prompt referral to an appropriate agency or government program if financial assistance is indicated; assist with re-certification papers to maintain status of government programs such as Medical Assistance Social Security and S.S.I.; notify Business Office.
• Completes assessments of residents as necessary for care planning MDS RAPS PPS and other required documents.
• Serves as resident advocate coordinates invitation of residents and families.
