Weoperatea collaborative network of more than 100 communitybased service providers across metropolitanregionaland remote Australia including Aboriginal and Torres Strait Islander communities partnering with individuals and employers to deliver positive longterm outcomes.

As part ofCoActsexciting organisational growth strategy led by an experienced NDIS General Manager we are expanding beyond traditional employment services and building a new NDIS business. This includes Support Coordination and Allied Health services withfuture plansto open a high school to better support individuals through wellbeingeducationand pathways to employment.

Our Support Coordination team will play a key role in delivering inclusive NDIS personcentred services that empower individuals to achieve meaningful sustainable outcomes.

The Role

CoAct is seeking aSupport Coordinator PracticeLeadto joinour new NDIS Support Coordination team. This role delivers highquality personcentred NDIS Support Coordination services to participants with complex and multiple support needs while also supporting strong practiceconsistencyand quality across the team.

The role supports participants to build capacity exercise choice and control and navigate the NDIS and broader service addition the Practice Lead contributes to effective service delivery byprovidingpractice guidance supporting quality outcomes mentoring team members and ensuring services are delivered in line with NDIS Practice Standards safeguardingexpectationsand organisational requirements.

The Support Coordinator Practice Leadoperateswith a high degree of autonomy managing complex caseloads coordinating multiagency supports and contributing to a culture of accountabilityqualityand continuous improvement.

In this role you will:

  • Deliver highquality NDIS Support Coordination services to participants with complex and highrisk needs
  • Support participants to understand and implement their NDIS plans promoting choice control and informed decisionmaking
  • Coordinate supports to maximise positive participant outcomes value for money and plan effectiveness
  • Manage and support complex and escalated cases including proactive risk identification and mitigation
  • Providepractice guidance mentoring and support to Support Coordinators to strengthen service quality and consistency
  • Contribute to qualitycomplianceand documentation standards across the team
  • Build andmaintainstrong relationships with service providers and key stakeholders

Skills and Experience

To be successful in this role you will bring:

  • Demonstrated experience delivering NDIS Support Coordination to participants with complex and multiple support needs
  • Sound knowledge of the NDIS NDIS Practice Standards safeguardingrequirementsand service systems
  • Experience managing complex or highrisk cases including proactive risk identification and escalation
  • Strong time management and workload prioritisation skills
  • Ability to build andmaintaineffective professional relationships with participants serviceprovidersand stakeholders
  • Experience supportingmentoringor guiding colleagues to achieve quality outcomes
  • A valuesdriven approach with a strong commitment to personcentred ethical practice

Qualifications

  • Relevant tertiary qualification (e.g. Social Work Community Services HumanServicesor a related discipline) or equivalent experience within disabilitycommunityor social services

Location

This role is based at ourStones Corner officeandoperatesunder ahybrid workfromhome arrangement.
