About the Role
We are seeking a compassionate and motivated Support Worker to support clients in this role you will provide psychosocial support assist with NDIS applications and helpclients build independence confidence and sustainable lifestyles.
Key Responsibilities
- Support clients to develop daily living and functional skills
- Provide emotional support guidance and problem-solving strategies
- Maintain accurate case notes and monthly reports
- Monitor client progress and follow up on referrals
- Work collaboratively with families service providers and stakeholders
Skills & Experience
- Certificate IV in Mental Health Community Services or related field (or relevant experience)
- Strong understanding of mental health and recovery-focused practice
- Experience working with diverse and vulnerable populations
- Excellent communication and interpersonal skills
- Ability to advocate for clients and support access to services
- Experience or willingness to facilitate group programs
Mandatory Requirements
- Current NDIS Worker Screening Check
- National Police Clearance
- Current Drivers Licence
- Working With Children Check (WA)
Desirable
- Experience working in an Aboriginal Community Controlled Organisation
