Job Summary
DUTIES AND RESPONSIBILITIES:
The Dietician or Nutritionist will split time between the Residential Program and the Article 16 clinic.
At the Article 16 clinic the Dietitian/Nutritionist will serve as a clinical provider promoting healthy lifestyle habits and helping clients make informed food choices within their abilities. The role will include nutritional assessment and Medical Nutrition Therapy (MNT). Providing clinical interventions for diabetes management weight gain/loss programs gastrointestinal disorders (constipation GERD) dysphagia diets (in conjunction with speech therapy) enteral nutrition management and any other appropriate dietary interventions.
The Dietician/Nutritionist shall be responsible for the assessment of each individual residing at an ACHIEVE home and for the subsequent development of treatment plans as appropriate.
- Assesses nutritional status needs and knowledge and self-feeding skill level for all individuals.
- Develop treatment plans to remediate deficits and/or enhance skills in nutrition and self-feeding for individuals. Implement these treatment plans directly or supervise program staff in their implementation as decided by the treatment team.
- Provides in-service training in conjunction with appropriate staff to develop nutritional awareness and knowledge of feeding programs in general and in regard to an individuals specialized needs.
- Provides initial follow-up diet counseling to individuals and to IRA staff as needed.
- Reviews and approves IRA menus.
- Monitors participants mealtime regarding:
- a. Appropriateness of meeting individual dietary needs.
- b. Proper eating skill techniques.
- c. Interpersonal mealtime behaviors whenever available.
- d. Monitors the appropriate supply of foods.
Responsible for addressing and reporting any concerns or suspected situations that adversely impact the health and safety of any individual to the Program Manager (PM) and Registered Nurse (RN).
- Responsible for complying with all waiver Part 633 and Part 624 regulations. Responsible for reporting any violations deficiencies and/or incidents to the Direct Supervisor as they are discovered or observed.
- Evaluates an individuals performance in treatment plans.
- Ensures program compliance with all pertinent health and hygiene codes that relate to nutritional services and food preparation and delivery (NYS Health Department Sanitary Code Subpart 14-1 Title 10 NYCRR).
- Participate in life plan and team meetings to ensure appropriate nutritional and self-feeding needs are addressed. Develops weight control programs for those individuals with an identified need. Participates in professional activities to maintain and enhance skills as dietician and other responsibilities as designated by supervisor.
- Consults with providers and the RN regarding diet orders and other nutritional concerns related to individuals enrolled in this program.
QUALIFICATIONS
A Bachelors Degree with major studies in food and nutrition from an accredited college or university and is registered by the American Dietetic Association. Minimum one years experience.

A combination of education and experience that meets the needs of the reviewing agencies.
- Possess a valid drivers license and good driving record acceptable to the agencys insurance carrier.
- Maintain certification in First Aid CPR SCIP and Medication Administration.
- Responsible for following agency safety and health policies procedures and rules.
Wage Range $29 - $45/hr
