Registered Nurse (PRN) Home Health

As a Home Health RN you will:

• Deliver skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care

• Perform comprehensive patient assessments including physical psychosocial and environmental evaluations

• Develop implement and update personalized care plans based on patient needs and progress

• Provide education and support to patients families and caregivers to promote independence and improve outcomes

• Monitor patient conditions and communicate changes to physicians Clinical Supervisors and care team members

• Coordinate care with an interdisciplinary team to ensure continuity and quality of care

• Accurately document patient visits assessments and care provided in accordance with company standards and regulatory requirements

• Ensure compliance with HIPAA and maintain strict patient confidentiality

• Promote a safe environment by identifying and addressing potential risks in the home setting

What Youll Need

• Current unrestricted Registered Nurse (RN) license in the state of practice

• Minimum of 1 year of clinical nursing experience (home health experience preferred)

• Strong assessment critical thinking and communication skills

• Ability to work independently and manage a patient caseload

• Willingness and ability to travel within assigned territory (50%)

• Valid drivers license reliable transportation and current auto insurance

• Ability to lift/move up to 50 - 100 lbs and assist with patient mobility

• Associate or bachelors degree in nursing (BSN preferred)

• Experience with HomeCare HomeBase (preferred)
