SUMMARY STATEMENT OF POSITION:

Under the direction of the Supportive Housing Supervisor the Supportive Housing Specialist develops individual recovery plans based on the supervisors assessment and CSB referral. The Supportive Housing Specialist engages in Housing First Principles to engage individuals with serious mental illness and assist in finding and keeping housing. This position is entirely in the community.

PRIMARY DUTIES:

  • Assesses stage of treatment stage of change skills and abilities of clients and engages clients in skills building activities that focus on recovery community integration and wellness.
  • Carries a caseload of 15.
  • Outreaches and engages those referred for housing assistance.
  • Provide individualized interventions to securing rental assistance paying rent adhering to lease obligations securing essential resources maintaining apartment conditions and finding other housing options if desired.
  • Develops a person-centered treatment plan and completes documentation of services. Reviews and modifies objectives and/or interventions in a timely manner.
  • Assist individuals to make informed decisions about recovery from mental illnesses and co-occurring disorders.
  • Routinely liaison and collaborate with other care-providers and/or treatment team.
  • Maintain electronic health record (EHR) as required by CARF Licensure and Medicaid.
  • Demonstrate flexibility in the performance of duties and/or other duties as assigned.
  • Represent staff in agency and/or community-based workgroups to enhance services provided within and related to mental health support services for target population as designated by supervisor.

QUALIFICATIONS:

  • Qualified Mental Health Professional (QMHP) preferred.
  • Bachelors degree in psychology social science or related field preferred.
  • Experience working with seriously mentally adults and housing.
  • Excellent written oral and interpersonal communication skills.
  • Possess knowledge and skill in computer procedures relevant to this position.
  • High degree of empathy toward individuals with serious mental illness.

ESSENTIAL COMPETENCIES:

  • Demonstrate sound knowledge of evidence-based practices.
  • Demonstrate the ability to monitor signs and symptoms of mental illness and/or co-existing disorders and strategies to effectively manage symptoms.
  • Engage with individuals to find and keep permanent supported housing.
  • Demonstrate the ability to problem solve and work collaboratively with Supervisor and team members.
  • Provide creative skills teaching opportunities based on client needs.
  • Demonstrated knowledge and skill in computer procedures relevant to this position.
  • Minor on call availability.

EXPECTATIONS REGARDING QUANTITY OF WORK

  • Manage a case load of 15.
  • All documentation will be completed within 24 hours of the service provided. Credible profiles and all paperwork will be current and reviewed/updated as needed at least quarterly. Quarterly Reviews must be done concurrently as they are due.
  • Meet productivity requirements as set by the agency.
