Job Type: Contract 13 Weeks
Shift: 8-Hour Shifts Flexible Schedule

About the Role:
Seeking a dedicated Physical Therapist to provide individualized rehabilitation services focused on improving mobility functional independence and patient quality of life.

Key Responsibilities:

• Evaluate musculoskeletal neurological and functional mobility status

• Develop individualized short- and long-term treatment goals

• Provide therapeutic exercises mobility training transfers and gait training

• Recommend and educate patients on assistive devices

• Reassess patient progress and modify treatment plans

• Collaborate with physicians and interdisciplinary healthcare teams

• Educate patients and caregivers on home exercise programs

• Participate in discharge planning and continuity of care

• Maintain accurate evaluations treatment notes and discharge documentation

• Assist in training staff students and therapy assistants

• Support quality improvement and compliance initiatives

Requirements & Preferred Skills:

• Bachelors degree in Physical Therapy or higher required

• Current Illinois Physical Therapist license required

• Strong knowledge of rehabilitation techniques and therapy modalities

• Ability to provide care across assigned patient age groups

• 12 years of Physical Therapy experience preferred

• Strong communication analytical and documentation skills

• Ability to work in interdisciplinary care settings

Physical Requirements:

• Ability to lift and carry up to 50 pounds

• Ability to stand for extended periods

• Frequent bending stooping pushing and pulling

• Ability to assist with patient positioning and transfers
