Job Summary
Job Title: Primary RN - Case Manager - Hospice
Location: Grand Junction CO 81506
Contract Length: 13 Weeks
Schedule: 40 Hours/Week
Position Summary
Seeking an experienced Hospice RN Case Manager to provide compassionate patient-centered care in a home hospice setting. The RN will coordinate care plans perform assessments educate families and collaborate with interdisciplinary teams to ensure quality end-of-life care.
Requirements
Active RN License
Hospice experience preferred
Case Management experience preferred
BLS Certification
Strong communication and documentation skills
Ability to work independently in field-based settings

Day shift schedule
Travel assignment
Mileage/field travel may be required
EMR experience preferred
