As a Pharmacist you will:

  • Evaluate physician medication orders for drug appropriateness correct dosage potential drug interactions and proper route of administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles including medical history lab results and current medications (prescription OTC and home remedies).
  • Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered reviewed and communicated to the healthcare team.
  • Collaborate with physicians nurses and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patients medical record including interactions with families and caregivers.
  • Implement and enforce policies and procedures for the safe handling dispensing and clinical oversight/management of medications.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy-related tasks and responsibilities as assigned.

Qualifications

Qualifications

Required:

  • Bachelors Degree in Pharmacy Pharm D preferred
  • One year experience in long term care desired
  • License/Certification(s): current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines medication safety practices and regulatory standards (CMS DEA FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement and effective written and verbal communication

Core Responsibilities (Operational Workflow)
1) Order Verification (Primary Function)
  • Review and verify medication orders in real-time
  • Ensure:
    • Appropriateindication
    • Correctdose route frequency
    • Renal/hepatic adjustments
    • Allergy and interaction screening
  • Release verified orders to ADCs (Omnicell/Pyxis) for nursing access
2) Clinical Review & Intervention
  • Perform prospective and retrospectiveclinical order review
  • Provide recommendations for:
    • Antibiotic stewardship (IDSA-aligned)
    • Renal dosing adjustments
    • High-risk medications (anticoagulants opioids insulin vancomycin AUC)
  • Communicate directly with providers and nursing staff
3) Remote Oversight of Medication Use
  • Monitor medication administration trends and discrepancies
  • Support nursing workflow:
    • Timing of doses
    • Missing medication resolution
  • Provide guidance on IV compatibility infusion rates and administration safety
4) Controlled Substance & Compliance Oversight
  • Review narcotic usage patterns and discrepancies
  • Support diversion monitoring efforts (DEA expectations)
  • Ensure:
    • Valid order present for all controlled substance dispensing
    • Documentation aligns with administration records
5) Regulatory & Survey Readiness
  • Maintain compliance with:
    • CMS Conditions of Participation (CoPs)
    • Joint Commission Medication Management (MM standards)
    • Texas State Board of Pharmacy (TSBP)
  • Support documentation and audit readiness (72-hour retrieval standard)

Required Qualifications
  • PharmD or BS Pharmacy degree
