As a Pharmacist you will:
- Evaluate physician medication orders for drug appropriateness correct dosage potential drug interactions and proper route of administration.
- Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
- Maintain up-to-date patient/resident profiles including medical history lab results and current medications (prescription OTC and home remedies).
- Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered reviewed and communicated to the healthcare team.
- Collaborate with physicians nurses and other healthcare professionals to provide clinical and technical support.
- Document all communications and clinical interventions in the patients medical record including interactions with families and caregivers.
- Implement and enforce policies and procedures for the safe handling dispensing and clinical oversight/management of medications.
- Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
- Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
- Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
- Perform other pharmacy-related tasks and responsibilities as assigned.

Qualifications
Required:
- Bachelors Degree in Pharmacy Pharm D preferred
- One year experience in long term care desired
- License/Certification(s): current and valid pharmacist license in the state of practice
- Knowledge of drug therapy guidelines medication safety practices and regulatory standards (CMS DEA FDA).
- Proficiency in electronic medical records and pharmacy information systems
- Strong clinical judgement and effective written and verbal communication

- Communicate directly with providers and nursing staff
- Monitor medication administration trends and discrepancies
- Support nursing workflow:
- Timing of doses
- Missing medication resolution
- Provide guidance on IV compatibility infusion rates and administration safety
- Review narcotic usage patterns and discrepancies
- Support diversion monitoring efforts (DEA expectations)
- Ensure:
- Valid order present for all controlled substance dispensing
- Documentation aligns with administration records
- Maintain compliance with:
- CMS Conditions of Participation (CoPs)
- Joint Commission Medication Management (MM standards)
- Texas State Board of Pharmacy (TSBP)
- Support documentation and audit readiness (72-hour retrieval standard)
- Active licensure in all practicing states (TSBP NM Board)
- Prospective drug utilization review (DUR) prior to dispensing (CMS CoPs)
- Controlled substance oversight (DEA compliance)
- Timely documentation and retrievability of pharmacy actions
- Antibiotic stewardship interventions (IDSA guidance)
- AUC-based vancomycin monitoring when appropriate
- Standardized remote verification workflows
- Clear communication loops with nursing and providers
- Experience with:
- Remote access platforms (VPN/Parallels)
- ADC systems (Omnicell Pyxis)
- Meditech Cerner CPSI / TruBridge or similar EHRs
- Ability to work independently with strong clinical judgment
- High attention to detail under low supervision environments
- Anticoagulants (heparin DOACs) bleeding risk dosing errors
- Opioids (Schedule II) diversion respiratory depression
- Insulin hypoglycemia risk
- Electrolytes (K Mg Na) cardiac/neurologic complications
- Vancomycin / Aminoglycosides nephrotoxicity
- PharmD or BS Pharmacy degree
- Active licenses:
- Texas (required)

- Minimum 2 years inpatient hospital experience preferred
- Strong clinical knowledge and independent decision-making ability
- Experience inremote pharmacy services
- Familiarity withcritical access hospitals (CAH)

- Pharmacokinetics (vancomycin AUC dosing)
