Position: Physical Therapist (PT)
Salary Range: $39.2 - $41.78 per hour
Hours: 30 hours per week Monday - Friday no weekend rotation

Job Summary:
We are seeking a highly skilled and motivated Physical Therapist to join a SNF facility on a full-time basis. The PT will be responsible for evaluating and treating patients with a variety of physical disabilities and injuries working closely with physicians and other healthcare professionals to develop and implement individualized treatment plans.
Responsibilities:
- Conduct thorough evaluations of patients physical abilities including range of motion strength balance and functional abilities
- Develop personalized treatment plans based on evaluation findings and patient goals
- Utilize a variety of treatment techniques including therapeutic exercises manual therapy and modalities to help patients improve mobility strength and function
- Monitor and document patient progress and adjust treatment plans as needed
- Educate patients and their families on exercises and home care techniques to promote continued progress and independence
- Collaborate with other healthcare professionals such as physicians occupational therapists and speech therapists to ensure comprehensive care for patients
- Maintain accurate and up-to-date patient records
- Stay current with advancements in the field of physical therapy through continuing education and professional development opportunities
Requirements:
- Bachelors Masters or Doctorate degree in Physical Therapy from an accredited program
- Current state PT license
- CPR/BLS certification
- Minimum of 1-2 years of experience in a clinical setting preferably in an outpatient or orthopedic setting
- Strong knowledge of anatomy physiology and kinesiology
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong time management and organizational skills
- Compassionate and patient-centered approach to care
