Responsibilities

• Provide in-home physical therapy services
• Develop individualized care plans
• Collaborate with interdisciplinary team members
• Document patient progress accurately and timely

Qualifications

• Active California Physical Therapist (PT) license
• CPR certification
• Valid drivers license clean driving record
• Reliable transportation & auto insurance
• Ability to work independently

Home health experience preferred; new grads welcome!

Pay

• Routine Visits: $69$79
• Evaluations: $91$104
• Admissions: $172.50$197.50

Actual pay rates vary depending on factors including type of visit type of experience location and years of experience.
