Registered Nurse Home Health

University Home Care Inc is seeking a compassionate and dedicated Registered Nurse Home Health to join our team in Livonia MI. This role involves providing high-quality nursing care to patients in their homes promoting health managing chronic conditions and supporting recovery. The ideal candidate will demonstrate excellent clinical skills strong communication abilities and a commitment to patient-centered care.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and care plans.
• Assess patients health status and needs develop individualized care plans and document progress.
• Administer medications treatments and therapies as prescribed.
• Educate patients and their families on disease management prevention and wellness.
• Collaborate with interdisciplinary healthcare team members to coordinate care.
• Maintain accurate and timely clinical documentation and comply with all regulatory requirements.
• Monitor patient safety and report any changes or concerns promptly.

Requirements

• Current valid Registered Nurse (RN) license in the state of Michigan.
• Minimum of 1 year of nursing experience preferably in home health or related field.
• Knowledge of home health care regulations and nursing best practices.
• Strong assessment clinical and organizational skills.
• Excellent interpersonal and communication skills.
• Reliable transportation to travel to patient homes in Livonia MI area.
