MDS Enrollment Registered Nurse

Full-Time Monday-Friday 8:00 AM-4:00 PM No Nights Weekends or Holidays

We are seeking a compassionate Registered Nurse (RN) to serve as an MDS Enrollment RN. In this role youll complete in-home clinical assessments for prospective participants and collaborate with an interdisciplinary team to support the enrollment process. • Conduct in-home assessments and complete the Minimum Data SetHome Care (MDS-HC)
• Evaluate medical functional psychosocial caregiver and home safety needs
• Present assessment findings to the interdisciplinary team
• Coordinate additional clinical evaluations as needed
• Serve as a clinical liaison between prospective participants caregivers referral sources and the care team
• Maintain accurate electronic documentation and support a seamless enrollment process

• Active Massachusetts Registered Nurse (RN) license
• Associates or Bachelors degree in Nursing or completion of an accredited Nursing Diploma Program
• At least 1 year of nursing experience in geriatrics home health long-term care case management community health or adult behavioral health preferred
• Strong assessment communication and organizational skills
• Experience with electronic medical records and Microsoft Office
• Valid drivers license and reliable transportation for frequent local travel

• This is a field-based position with frequent local travel
