Job Overview:
Care staff are the backbone of the organization and the first last and most frequent point of contact with residents each day. As a caregiver with us you will assist the resident in maintaining their independence and quality of life by performing tasks and duties to support the residents physical and psychological health. Additionally youll promote the Harvest Homes culture by fostering resident engagement and belonging encouraging individual choice and creating opportunities where residents can experience purpose and self-fulfillment. You must have strong interpersonal skills effective communication and an empathetic understanding of the resident and their history and this role you must be people-oriented self-directed and a builder of authentic relationships.
Compensation Range:
Caregiver$18.00 to $18.50 (based on experience level)
Shift: Monday-Friday 2pm-10pm
Thursday-Friday 2pm-10pm
Location: 6921 N Roberts Ave. Portland Oregon 97203
Job Responsibilities (not limited to):
- Deliver services and assist with Activities of Daily Life (ADLs) as outlined in a residents Service Plan (SP) with respect empathy competence and encouragement throughout the residents day.
- Maintain patient stability by monitoring vital signs and weight and document as necessary.
- Assist with ADLs such as dressing showering toileting incontinence care eating and grooming.
- Follow and consistently build the simple routines and organizational structures of the community.
- Actively participate in the community with the residents families and other staff.
- Document detailed reporting of the ADL care completed and any observations made in the electronic portal MARs or handwritten reports etc. as required.
- Identify and promptly report changes in resident health status and/or daily routine in the Daily Team huddle to the RCC and RN/LPN and in the Progress Notes.
- Attend and participate in daily team huddles and pass downs staff meetings family night service planning meetings etc.
- Keep up-to-date on required training and in-service hours (not less than 12 hours per year for ALF and 16 hours per year for RCF) including Food Handlers card First Aid (CPR if required) dementia training and other state and company-mandated trainings.
- Comply with HIPAAby protecting confidential patient information.
- Follow Harvest Homes procedures and adhere to the guidelines set forth for processes that have been put in place.

Qualifications
- Must be 18 years of age or older.
- High school diploma or equivalent.
- Value and respect of elders.
- Strong desire to work with seniors.
- Speak read and write English.
- Excellent oral communication.
- Good reading comprehension skills.
- Good written communication with the ability to correspond and document properly in charts and eMAR
- Able to turn and move individuals who require assistance.
- Able to stay on feet and remain active throughout the shift.
- Must be able to bend stoop twist reach and squat often throughout the shift.
- 50-pound lifting requirement.
