The Physician Assistant will provide comprehensive primary care services to patients across the lifespan including preventive care chronic disease management and acute care. This role emphasizes continuity of care evidence-based practice and collaboration with physicians and multidisciplinary teams in a supportive clinical environment.
Key Responsibilities:
- Provide comprehensive primary care services including physical exams diagnoses and treatment planning
- Manage acute and chronic conditions such as diabetes hypertension and respiratory illnesses
- Perform routine procedures and preventive screenings
- Prescribe medications and therapies in accordance with state regulations and supervision guidelines
- Collaborate with physicians and specialists for coordinated patient care
- Educate patients on health maintenance disease prevention and lifestyle modifications
- Maintain accurate and timely documentation within the EMR
- Participate in quality improvement initiatives and population health programs
