Tuesday: 9:00 AM 5:00 PM
Wednesday: 10:00 AM 3:00 PM
Position Overview
Navitas Healthcare LLC is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join a dynamic medical social services team for a 27-week assignment. This role supports patients and families in a hospital or outpatient setting focusing on psychosocial assessments care coordination and discharge planning. Experience in transplant services is a plus but not required.
Key Responsibilities
Conduct comprehensive psychosocial assessments for patients and families
Develop and implement individualized care and discharge plans
Provide emotional support counseling and crisis intervention
Collaborate with multidisciplinary healthcare teams to coordinate patient care
Connect patients with community resources and support services
Advocate for patient needs and ensure continuity of care
Maintain accurate and timely documentation in compliance with regulatory standards
Support patients with chronic illness complex conditions or transplant-related care
Participate in case conferences and interdisciplinary team meetings
Qualifications
Masters Degree in Social Work (MSW) Required
Active Licensed Master Social Worker (LMSW) license in the State of New York (or ability to obtain within one year of hire)Required
Prior experience in a hospital or outpatient medical setting Required
Strong knowledge of psychosocial assessments and care planning
Excellent communication interpersonal and organizational skills
Ability to work effectively within a multidisciplinary team
