Schedule: Day Shift 24 Hours/Week (3 Days)
- Monday: 9:30 AM 4:00 PM
- Tuesday: 9:00 AM 5:00 PM
- Wednesday: 10:00 AM 3:00 PM
Position Overview
Navitas Healthcare LLC is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join a dynamic medical social services team for a 27-week assignment. This role supports patients and families in a hospital or outpatient setting focusing on psychosocial assessments care coordination and discharge planning. Experience in transplant services is a plus but not required.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement individualized care and discharge plans
- Provide emotional support counseling and crisis intervention
- Collaborate with multidisciplinary healthcare teams to coordinate patient care
- Connect patients with community resources and support services
- Advocate for patient needs and ensure continuity of care
- Maintain accurate and timely documentation in compliance with regulatory standards
- Support patients with chronic illness complex conditions or transplant-related care
- Participate in case conferences and interdisciplinary team meetings
Qualifications
- Masters Degree in Social Work (MSW) Required
- Active Licensed Master Social Worker (LMSW) license in the State of New York (or ability to obtain within one year of hire) Required
- Prior experience in a hospital or outpatient medical setting Required
- Strong knowledge of psychosocial assessments and care planning
- Excellent communication interpersonal and organizational skills
- Ability to work effectively within a multidisciplinary team
- Experience in transplant social work Preferred
