Position Summary
We are seeking an experienced Licensed Master Social Worker (LMSW) to provide comprehensive social work services within a healthcare setting. This role focuses on patient assessment discharge planning and collaboration with interdisciplinary teams to ensure optimal care outcomes.
Key Responsibilities
- Conduct interviews with patients and families to assess social psychological and environmental needs
- Review medical records and collaborate with healthcare teams to determine appropriate care plans
- Lead discharge planning for patients requiring specialized care including coordination with rehabilitation or extended care facilities
- Assist with patient placement including pediatric cases when applicable
- Participate in interdisciplinary meetings and communicate key social and emotional factors impacting patient care
- Support goals-of-care discussions and end-of-life planning when appropriate
- Collaborate with staff to support quality improvement initiatives
Required Qualifications
- Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)
- Experience in inpatient hospital social work and discharge planning
- Experience with goals of care discussions and end-of-life planning
- Exposure to oncology patient populations preferred
- Strong knowledge of psychosocial assessment
- Valid Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED)
Education
- Masters Degree in Social Work (MSW)
