Key Responsibilities

• Provide specialist palliative care to patients with complex clinical and psychosocial needs
• Manage and coordinate a defined patient caseload ensuring high-quality individualized care
• Offer expert advice on pain and symptom control including physical emotional and spiritual support
• Collaborate with multidisciplinary teams to plan implement and evaluate patient care
• Support patients and families in discussing and planning end-of-life care preferences
• Deliver education mentoring and training to healthcare staff to enhance palliative care practices
• Participate in clinical audits quality improvement initiatives and service development
• Act as a key link between hospital services and community-based care providers

Requirements & Qualifications

• Registered Nurse qualification with valid professional registration
• Bachelors degree in Nursing (or equivalent)
• Postgraduate qualification in a relevant specialty (or willingness to complete)
• Significant post-registration clinical experience
• Proven experience in supportive and palliative care
• Strong communication leadership and decision-making skills
• Experience in teaching mentoring and knowledge sharing
• Ability to manage complex patient needs and challenging clinical situations
• Experience in audit research or quality improvement is an advantage
• Prescribing qualification (desirable but not essential)
