Key Responsibilities
Provide specialist palliative care to patients with complex clinical and psychosocial needs
Manage and coordinate a defined patient caseload ensuring high-quality individualized care
Offer expert advice on pain and symptom control including physical emotional and spiritual support
Collaborate with multidisciplinary teams to plan implement and evaluate patient care
Support patients and families in discussing and planning end-of-life care preferences
Deliver education mentoring and training to healthcare staff to enhance palliative care practices
Participate in clinical audits quality improvement initiatives and service development
Act as a key link between hospital services and community-based care providers
Requirements & Qualifications
Registered Nurse qualification with valid professional registration
Bachelors degree in Nursing (or equivalent)
Postgraduate qualification in a relevant specialty (or willingness to complete)
Significant post-registration clinical experience
Proven experience in supportive and palliative care
Strong communication leadership and decision-making skills
Experience in teaching mentoring and knowledge sharing
Ability to manage complex patient needs and challenging clinical situations
Experience in audit research or quality improvement is an advantage
Prescribing qualification (desirable but not essential)
