Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

The TERROS GMH/SA Discharge Planners serve as a liaison between the inpatient
setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient
Providers to review presenting clinical information.
Duties may include:
- identify resources for successful discharge to the community and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider.
- Discharge Planner activities will be performed in both inpatient and outpatient settings. The DischargePlanner will connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge. The Discharge Planner will work with BHRs while in the hospital will work with transitioning BHRs the hospital unit staff the Outpatient team family RBHA care management PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services reduce the risk of readmission to hospital reduce the utilization of Crisis services and improve the coordination with the PCP and Terros psychiatric provider.
Qualifications
- Must have a high school diploma or GED
- Most possess a combination of behavioral health education and work experience totaling a minimum of four (4) years; or a bachelors degree in any field plus one year of work experience in a behavioral health service delivery or a bachelors degree in a behavioral health field
- Must have valid Arizona drivers license be 21 years of age with minimum 4years driving experience and
meet requirements of Terros Healths driving policy - Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card
(Level 1) within 7 working days of assuming role. - Must pass a TB Test.
