Job Summary
Position Summary
The Perinatal Co-Occurring Outpatient Therapist will provide timely clinical interventions among women within a perinatal gender-specific trauma-responsive outpatient and OP-housing treatment program. This role is responsible for delivering individualized evidence-based screenings and assessments SUD /MH diagnoses and implementing clinical findings to ensure perinatal and parenting care successfully reaches the appropriate level of care. This person will be responsible for clinical charting and documentation and working within several interdisciplinary clinical team meetings ( UNC Reach Wake County Health Department OB Care Nurses UNC Rex UNC Johnston Health UNC Perinatal Outpatient Psychiatry and Perinatal and Parenting programs for Women in North Carolina). This person will be responsible for assisting with enrollment during holidays and office closures to ensure continuity of care. Key Responsibilities: Complete comprehensive clinical assessments including substance use mental health and biopsychosocial evaluations to determine medical necessity and appropriate level of care Develop and implement individualized Person-Centered Plans (PCPs) in collaboration with the client and treatment team Coordinate care with a multidisciplinary treatment team including case management peer support medical and psychiatric providers Ensure timely accurate and compliant electronic documentation in accordance with NC Medicaid and state regulatory requirements Support discharge and transition planning to ensure continuity of care and linkage to community-based services Support on-call clinical coverage such clinical assessments and enrollment during Holidays and office closures *LCSW LCAS LMFT or LCMHC or provisional license
