Job Summary

Summary

The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients outpatients and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination skilled at managing multiple priorities at once and engaging in creative compassionate and ethical problem-solving. As a member of a multidisciplinary health team they perform comprehensive assessments and develop treatment plans that integrate the medical social and resource issues that impact individual patients families and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

Full time outpatient therapist opportunity embedded within a primary care pediatric clinic. Our social worker will work as a member of the pediatric primary care department to provide short term therapy to patients with clinical mental health conditions including but not exclusive of anxiety depression and other mood related conditions as well as ADHD and behavioral dysregulation.

Conducts psychosocial assessments provides clinical services to pediatric patients that address psychosocial environmental age specific and cultural issues in order to maximize emotional social and physical wellbeing and the effective/efficient use of health care and community resources. Available to Pediatricians for consultations and acute assessments during routine clinic encounters.

Experience and training with elementary aged (5-11) children as well as CBT preferred. Works effectively as part of an interdisciplinary health care team and is comfortable referring to and collaborating with interdepartmental specialists and programs outside agencies and other relevant community resources.

Essential Functions
Provides psychosocial assessments of patients and families with social emotional interpersonal and/or environmental issues.
Formulates biopsychosocial assessment disposition and treatment plans.
Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.
Providing short-term evidence-based psychotherapy (individual groups) to patients in the pediatric practice in-person and virtually
Screen patients and develop comprehensive assessments and treatment plans for patients engaged in BH treatment
Providing a range of interventions to support primary care mental health and behavioral needs such as crisis intervention; care coordination; information; referrals; and safety planning.
Assessing patients presenting with new mental health and behavioral symptoms to aid in the identification of the best treatment options for them
Participating in formal and informal case reviews seminars and / or inter- and intra-departmental peer review meetings
Working closely with the other members from the primary care practice to provide timely and appropriate care
Advocate for school related needs as applicable to behavioral health diagnoses.
