P-Physical Therapist

Position: Physical Therapist (P-Physical Therapist)

Salary Range: $41.00 to $50.00 per hour

Hours: 35 hours per week Monday-Friday with weekend rotation

Facility: Small Continuing Care Retirement Community (CCRC) with a 58-bed Skilled Nursing Facility (SNF)

Job Summary:

We are seeking a skilled and compassionate Physical Therapist (P-Physical Therapist) to join our team at our small CCRC with a 58-bed SNF. The P-Physical Therapist will provide physical therapy services to our residents in both short term and long term care settings as well as outpatient services for our Assisted Living and Independent Living residents.

Key Responsibilities:

- Conduct initial evaluations and develop individualized treatment plans for residents based on their specific needs and goals

- Administer prescribed physical therapy treatments including therapeutic exercises manual therapy and modalities

- Monitor and document residents progress adjusting treatment plans as needed

- Communicate regularly with the interdisciplinary team to coordinate care and ensure the best outcomes for residents

- Educate residents and their families on proper techniques and exercises to improve their mobility and function

- Participate in care conferences and discharge planning for residents

- Adhere to all state and federal regulations and professional standards in the delivery of physical therapy services

Qualifications:

- Current state license to practice as a Physical Therapist

- Minimum of 1 year of experience in a skilled nursing facility or similar setting preferred

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team

- Knowledge of Medicare guidelines and reimbursement for physical therapy services

- Experience with electronic medical records preferred
