Job Overview
The CommunityHealthWorker is part of Rural Health Connect South Jersey (RHCSJ)a community-based prevention programwith the goal ofidentifyingand engagingunisuredand underinsured rural residents of Atlantic Cape May Cumberland and Salem Counties. The RHCSJ team will conduct health screenings and connect individuals and families topreventative physical health behavioralhealthand family support services. Through community outreach and engagement the program intends to improve early identification of health risks throughscreeningsincrease access toservicesand reduce unmethealthneeds among this population.
As an integrated healthorganizationAcendavalues the use of evidence-based practice or EBPs.
KeyResponsibilities:
Assistin conducting direct outreach and presenting program services to rural communities local churches and other organizations that serve rural areasin Atlantic Cape May Cumberland and Salem Counties.
Identifyengageand connect withuninsured and underinsured individuals and familiestoconduct health screeningsusingCommCareplatform.
Provide resources and linkages to those uninsured and underinsured individuals and families who have physicalbehavioraland social health needs.
Help to organize and host community forums onpreventative healthcaresocial determinants of health risk factorsandprotective factors
Complete required client documentation and follow upswithin definedtimeframes
Work collaboratively with Community Care Partnersand external stakeholders to expand resources available to community members.
Meet regularly with the CHW Project Supervisor and attend meetings with Local Public Health designated staff when applicable
Requirements:
New Community Health Workers (CHWs) will be requiredtoattend the New Jersey Department of Health Colette Lamothe Galette Institute training and coursework
Must have shared life experiences in the communities they serve with shared values to support as well as become an intermediary link between health/social services and the community in order tofacilitateaccessto services and improve the service
deliverysystem.
Understand the impact of cultural diversity cultural humilityandcultural responsiveness training
Promote an environment of care which is inclusive and respectful of every internal and external stakeholders
Effective problem-solving organizational and time management skills
Decision-making priority setting and work delegation abilities
Administrative/Technologicalskills including Microsoft Office (Excel Word PowerPoint Access) and Outlook
Excellent knowledge of available community services and resources
Ability to work independently or in groups meet strict deadlines and manage or direct group activities
Ability to attend evening and weekend meetings
Strong communicationandinterpersonal skills
Comfortable with public speaking and facilitatingeventsin the community
Maintain a high levelof confidentiality
Excellent customer-focused interpersonal skills to interact effectively with health care practitioners the interdisciplinary health care team community agencies patients and families with diverse opinions values and cultural ideals
Must have a Valid NJ drivers license and own an insured car; position requires some travel within the counties
Must be able to work legally in the United States
Education/Training:
High-school degree or equivalency (GED) required
Associates or Bachelorsdegreein related field preferred
Prior and relevant professional experience working in the community
Internships may be substituted as work experience
Proof ofcompletion of Community Health Workers training (New Jersey Department of Health Colette Lamothe Galette Institute IMPaCTCareorrelevant certification).
Additional Information:
Base Pay: $22.60
