Role Scope:

The Referral Coordinator positionis responsible foraccurateandtimelydata entry of referral requests into the appropriate are also responsible for follow-up on all specialty referrals and coordination of care for urgent referral coordinator is an integral part of the care team working as a liaison between the patient providersutilizationmanagementand various departments to ensuretimelyaccess to specialty care.

  • Continue to raise the sharefeedback andhelp us implement changesin order toimprove clinical outcomes and experience for participants.
  • Exhibit and honor Habitats values.
  • Receive review and process referral requests from healthcare providers.
  • Coordinate with specialists hospitals and other healthcare providers to schedule participantappointments.
  • Ensure all referral information isaccurate complete and compliant with healthcare regulations.
  • Serve as a primary point of contact for participantsprovidersandspecialitynetwork
  • Provide participantswith information on referral processes appointment details andcollaborate withcareteamonany required pre-appointment preparations.
  • Maintain effective communication with healthcare providers to ensuretimelyupdates on referral statuses.
  • Verify and update participantrecords in the electronic health record system.

  • Prepare andsubmitreports on referral activities and outcomes.
  • Assistparticipantswith understanding theirPACEbenefits related to referrals.
  • Participate in provider relationsorjoint operations meetingsasrequested.
  • Performs related duties as assigned.

Required Qualifications:

  • High school diploma or GEDrequired.
  • Minimum of 2 years of experience.
  • Strong organizational and multitasking abilities.
  • Excellent communication and interpersonal skills.
  • Proficiencyin using EHR systems and other healthcare-related software.
  • Knowledge of medical terminology insurance processes and healthcare regulations.
  • Reliableandtimelytransportation to and from primary worklocationand other locationsto fulfill responsibilities will be provided according tocompanymileagepolicy.

Preferred Qualifications:

  • Associates or Bachelorsdegree in healthcare administration medical assisting ora relatedfield preferred.
  • Medical Assistant certificate.
  • Bilingual: Spanish/Englishpreferred.
  • A state issued drivers license personal transportation and auto insurance as required by law.

Compensation:

Wetake into accountan individuals qualifications skillset and experience role is eligible for medical/dental/vision insurance short and long-term disability life insurance flexible spending accounts 401(k) savings paid time off and company-paid holidays. The expected salary range for this position is $22.00 -$25.00 and is bonus eligible. The actual offer will be at the companys sole discretion anddeterminedby relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
