Job Summary
Social Services Agency seeks Community Support Professional to Supports individuals with disabilities in becoming valued members of their communities. Supports behavior that enhances inclusion in their communities. Provides direct training and support in individuals home and/or community. Empowers individuals and their family by providing the support and information necessary to make informed choices and decisions. Helps build the selfesteem and assertiveness necessary to lead a self-determining life. Use formal and informal methods to learn about the needs interests and desires of individual. Assists individuals in identifying the formal and informal support available in his/her community family and social networks and works with the individual to develop and sustain relationships with friends family and community members. Assists the individual in managing everyday aspects of life by utilizing habilitative strategies and providing support in a manner that builds on individual strengths and capabilities and maximizes choices. Pursues opportunities for professional growth and shares knowledge and resources with others. Assists the individual to overcome challenges through effective advocacy and the facilitation of self-advocacy. Use appropriate and safe strategies for crisis prevention intervention and follow up. Actively contributes to advancing the mission goals and organizational life of AHRC as well as individual he/she supports. Completes necessary documentation in an efficient and timely manner. Ensures that program remains in compliance with OPWDD regulations agency philosophies and policies. Facilitates effective communication with employees and outside agencies for ongoing development of community opportunities. Participate in any special case conference staff and parent meetings as appropriate.
Qualifications :
Minimum Requirements: bachelors in communication social communication or related field. Will accept a single degree/combination of degrees diplomas or professional credentials determined to be equivalent to a US bachelors in in communication social communication or related field by an accredited credential evaluation service.
