Position Overview: Promise Program has an immediate opening for a BHCMII (Behavioral Health Case Manager II) to support the PROMISE Care Management initiative. This role provides behavioral health case management services to individuals with mental health and substance use disorders ensuring coordination of care treatment planning and community resource navigation.
The PROMISE Care Manager works collaboratively with clinical teams community partners and clients to promote recovery independence and long-term stability.
Job Description:
The incumbent will work as part of an interdisciplinary team in the PROMISE program serving individuals with severe and persistent mental illness including those with co-occurring disorders. Incumbent will act as a Care Manager in performing assessments helping develop individualized recovery plans determining appropriate level of care placing individuals in those levels of care and then ensuring that care is delivered according to standards. Other duties will include visiting beneficiaries in the community charting and acting as liaison with a large network of community providers.

Position Responsibilities:
The BHCMII PROMISE Care Manager will be responsible for but not limited to the following:

Case Management & Care Coordination
-
Conduct comprehensive assessments of clients behavioral health needs strengths and barriers to recovery.

Develop implement and monitor individualized care plans aligned with PROMISE program requirements.
-
Coordinate care across multiple systems including mental health substance use primary care housing employment and social services.
-
Client Engagement & Support
-
Provide ongoing support and advocacy to clients with serious mental illness (SMI) and co-occurring substance use disorders.
-
Utilize motivational interviewing trauma-informed care and recovery-oriented practices.
-
Assist clients in developing independent living skills and accessing community resources.
-
Collaboration & Communication
-
Collaborate with multidisciplinary teams including psychiatrists therapists primary care providers and community partners.

Serve as a liaison between clients families and service providers to ensure continuity of care.

Participate in team meetings case conferences and care coordination rounds.
-
Documentation & Compliance
-
Maintain accurate timely and compliant clinical documentation in Electronic Health Records (EHR).
-
Ensure all documentation meets DSAMH Medicaid and other regulatory standards.

Complete required assessments progress notes and outcome measures as per program requirements.

Crisis Intervention & Safety Planning
-
Provide crisis intervention and de-escalation support as needed.
-
Develop and update safety plans in collaboration with clients.
-
Coordinate with emergency services when necessary.
