Job Summary

Starting Rate:$27.25 - $31.75 - Based on Experience Shift Differential and Weekend Differential available!!!!

Sign-On BONUS - Earn up to $5000

DESCRIPTION:

  • Manages a designated group of nursing employees on a nursing unit and delegates/supervises resident care.
  • Administers medications/treatments on the assigned unit

DUTIES ANDRESPONSIBILITIES:

  • Works closely with the Nurse Manager/Nursing Supervisor in all aspects of resident care to ensure continuity of resident care.
  • Administer medications and treatments in accordance with physician orders and facility protocols
  • Monitor and document residents health status reporting changes to the RN or attending physician promptly
  • Perform skilled nursing tasks including wound care catheter care and tube feedings
  • Communicate clearly with residents families and interdisciplinary team members
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Follow all state federal and facility regulations including infection control and HIPAA standards
  • Supervises CNAs
  • Checks medication/treatment supplies and reorders as necessary.
  • Assists in emergency care as necessary.
  • Assists the Nurse Manager/Nursing Supervisor with evaluation of residents needs condition and care.
  • Relieves CNAs for breaks and mealtime when necessary.
  • Monitors and assists residents during meal times
  • Follows proper policy and procedure with maintenance of controlled substances (i.e.: administration count documentation of effectiveness).
  • Assists the CNAs with resident care as needed.
  • Other related duties that may have become necessary or as directed by the supervisor department head and/or administration.

Qualifications:

  • Current L.P.N. licensed in the State of New York
  • Current CPR/BLS certification
  • Strong clinical assessment and communication skills
  • Ability to work collaboratively in a fast-paced team-oriented environment
  • Experience in a long-term care or skilled nursing setting preferred
  • CPR certification required
