Job Summary
Summary:Under direction of the Nurse Manager Clinical Coordinator & Charge Nurse the Community Residence Counselor
(CRC ) provides direct patient care and participates in therapeutic milieu within the scope of defined practice.
-Provides and supervises community activities according to needs.
-This may include driving & transporting patients to and from activities in hospital vehicles.
-Remains awake and alert at all times while on the job.
-Assists patients in maintaining and keeping various appointments.
-Clear concise and timely documentation and shift notes.
-Establishes relationships with patients that are helpful and maintains professional boundaries.
-Participates in patient rounds status checks or teams per program guidelines.
-Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
-Works effectively to manage the therapeutic milieu.
-Provides patients with direction and supervision as needed.
-Follows established hospital & program policies procedures and protocols.
-Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
-Communicates observations related to medication side effects physical and emotional changes to appropriate medical staff in a timely manner.
-Encourage patients in their activities of daily living.
-Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
-Completes mandatory annual in-service requirements including but not limited to: CPR and de-escalation.
-Attends and participates in rounds and case conferences when appropriate.
-Physical ability for participation in patient activities.
-Other duties as assigned.
The Behavioral Health Partial Hospital Program located on McLeans Belmont campus treats adults ages 18 struggling with mood and anxiety disorders depression personality disorders bipolar spectrum disorders and thought disorders. The approximate length of participation is three weeks (15 consecutive business days). The program is available in person only and operates Monday through Friday from 8:30am to 3pm.
The BHP emphasizes group therapy structure and skill building with intensive daily treatment programming designed to reduce symptoms and meet individual treatment needs. Each patient undergoes a thorough initial assessment to create a personalized care plan based on specific diagnoses and symptoms aiming to support stabilization and tailored treatment goals.
Clinicians help patients build coping skills using cognitive behavior therapy (CBT) dialectical behavior therapy (DBT) and acceptance and commitment therapy (ACT). We also integrate the systems model focusing on patient-family and patient-community relationships as well as positive psychology to enhance strengths and well-being.
In addition to group therapy treatment includes medication consultations individual therapy case management and aftercare planning.
