Greenfield Rehabilitation Agency is expanding and seeking passionate Occupational Therapists (OTRs) to join our exceptional team at our senior campuses in Lancaster while also supporting communities in Prairie du Chien. At Greenfield Rehab we provide functional effective and rewarding occupational therapy across skilled nursing facilities senior communities assisted living environments and in-home care.

This is a unique opportunity to combine hands-on clinical care with flexible schedulingwhether youre seeking full-time part-time or PRN workwhile making a real difference in the lives of older adults across multiple communities. Full-time candidates are eligible for a sign-on bonus of up to $6000.

Occupational Therapist (OTR) Responsibilities

• Complete functional and cognitive evaluations

• Develop individualized resident-centered treatment plans

• Provide skilled OT interventions to promote independence and quality of life

• Educate residents families and staff on adaptive strategies and safe mobility

• Recommend assistive equipment and environmental modifications

• Lead therapeutic activities to enhance coordination strength and functional abilities

• Collaborate with nursing PT SLP and interdisciplinary teams

• Maintain accurate and timely clinical documentation
