Job Overview:

TheTransition Age Youth (TAY) Forensic Specialistwill provide direct and indirect services for20-30young adults(ages16-24)involvedwithboth the Transition Age Youth (TAY) Initiative and the Criminal Justice System. This is a highly collaborative team approach position that worksclosely with the County TAY Providersas well as the Behavioral Health and Criminal Justice role provides assessment court advocacy service planning andcarecoordination from arrest through community reintegration.

Key Responsibilities:

ClientEngagement

  • Review referrals todetermineintercept point(courtjailcommunity)

  • Conduct outreach andmaintainmonthlyin-person contact

  • Engage clients in Allegheny County Jail and community settings

Assessment and Service Planning

  • Complete ANSA assessments within requiredtimeframe

  • Developand implementCourtService Planswith multidisciplinary teams

Court Involvement

  • Attendclient involvedcourt hearings

  • PresentCourtService Plansandclient progress

Care Coordination & Resource Linkage

  • Linkclients to mental health and drug & alcoholservices

  • Assistwith accessing servicesincluding:employment education housing benefits medication clothing identification etc.

  • Collaboratewith TAY Initiative Providers and treatment teams

Documentation& Compliance

  • Completeassessments service plans casenotesand encounter formswithin specifiedtimeframes

  • Meetestablishedproductivityandqualitystandards

  • Comply withHIPAA confidentiality policies

Professional Development

  • Participation in individual supervisionagencymeetings and ongoing trainings

Required Skills and Qualifications

  • Bachelors degree in Social Work Psychology Education Criminal JusticePolitical Science Sociology Social Welfareor a related field

  • 3years ofdirect clientexperience ina behavioralhealth Juvenile Justice or Criminal JusticeSystem orhumanservicessetting

  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines

  • Excellent communication and interpersonal skills with the ability to work independently and as part of a multidisciplinary team

  • Ability to navigate multiple electronic health recordsand Microsoft Office Products

  • Demonstrated cultural competence and the ability to work with diverse populations

Additional Requirements

  • Full-time position requiring40 hoursper week

  • ValidPennsylvania drivers license with access topersonalvehicle

  • Current auto insurance that meets minimum collision coverage requirements

  • Act 33 Act 34 and FBI Clearances (including Institutional Clearancesthrough the National Crimes Informational Center)

  • Ability to meet with clients inside Allegheny County Jail

  • Ability to completerequiredonboardingtraining within30 daysof hire

Work Conditions and Benefits:

  • Full-time position(40hoursper week) with occasional evening or weekend hours to meet client needs.

  • Hybrid work model(up to 2 days a weekafter probationary period): Field-based with theoptionto complete administrative tasks remotely.

  • Compensation:$24.52/hour totaling $51000 per year

  • Time Off:start with18 daysof paid time off5 sick days and10 holidays annually
