We are seeking a highly motived RN with a heart for patient advocacy and a passion for building professional relationships to join our growing team.
Key Responsibilities
- Build and grow relationships with hospitals rehab centers the VA and assisted living facilities
- Collaborate with hospital and rehab discharge planners case managers and physicians to identify patients suitable for home care.
- Provide education to patients and families about our home care services.
- Conduct clinical assessments to determine eligibility and service needs.
- Represent Senior Helpers within the local senior community at events and programs
Qualifications
- Currently licensed as an RN
- Experience in discharge planning or case management strongly preferred
- Strong interpersonal and communication skills
- Highly organized self-motivated and goal-oriented
- Valid drivers license and reliable transportation
Compensation
Total Compensation: $65000 $110000 (depending on license and experience)
Bonus Structure: Performance-based bonuses tied to referrals and admissions
