Job Overview:
We are seeking a licensed Physical Therapist (PT) for a 13-week contract at Eastern Montana Veterans Home. This role involves providing high-quality rehabilitative care in a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) setting focusing on improving patient mobility strength and overall functional independence.
Key Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic interventions to restore function and mobility
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely documentation
- Educate patients and caregivers on therapy techniques and goals
- Collaborate with interdisciplinary healthcare teams
Requirements:
- Active Montana Physical Therapist license (required)
- Minimum 1 year of PT experience
- Experience in SNF or LTC settings preferred
