Summary:
We are seeking a highly skilled and detail-oriented Oncology Pharmacist to join our healthcare team. This role is critical in ensuring the safe effective and compliant preparation and management of chemotherapy treatments while collaborating closely with oncology providers and nursing staff.
Qualifications-
Registered Pharmacist currently licensed in New York State

Completion of a PGY2 Oncology Residency and/or Board Certification in Oncology (BCOP) preferred

Alternatively 1 5 years of experience in preparing and evaluating chemotherapy regimens required

Strong familiarity with USP < 797> USP < 800> and other regulatory standards for sterile compounding

Excellent clinical judgment communication and organizational skills

Partner with Pharmacy Buyer to maintain an accurate patient list and appropriate par levels for chemotherapy agents updated weekly to minimize waste

Regularly review pharmacy inventory to ensure proper stock levels and organization

Remove expired or discontinued medications and promote adherence to the formulary system

Review all chemotherapy orders for accuracy safety and clinical appropriateness

Compare regimens against NCCN guidelines or clinical study protocols and attach supporting references

Collaborate with providers to round doses to optimal vial sizes to reduce waste

Verify appropriate diluents concentrations and stability parameters

Establish and document beyond-use dates coordinating updates within the drug information system

Monitor cumulative lifetime doses for applicable therapies

Communicate with oncology providers and nursing staff to clarify or resolve any medication-related questions or discrepancies

Actively participate in interdisciplinary discussions to optimize patient care

Coordinate with the Oncology Department on chemotherapy prescribing practices

Support medication reconciliation processes to ensure continuity and safety of care
