Overview

We are looking for a Pharmacist to join our team of providing the highest quality of care reliability and support to our patients caregivers and this role the Pharmacist will be responsible for dispensing medications providing clinical advice as well as delivery services.

Schedule:
Monday - Friday 
10:00am - 6:00pm
possible weekend rotation and on-call

Responsibilities

Provides clinical consultations to patients as well as recommendations to nurses physicians and other hospice personnel
Evaluates medication profiles compliance and modifications to achieve desired therapeutic outcomes
Answers customer questions and resolves customer issues in a timely manner
Retrieves data in the pharmacy system
Manages medication orders
Ensures proper storage and security conditions for medications
Maintains a productive orderly and error-free work environment

Qualifications

Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
Current and good standing Pharmacist License
Strong communication skills customer service and problem-solving skills
