Job Summary
Responsibilities:
Perform comprehensive nursing assessments addressing physical emotional psychosocial and spiritual needs.
Collaborate with the interdisciplinary team to develop implement and evaluate personalized care plans.
Provide skilled nursing interventions including wound care medication management IV therapy symptom/pain control and patient/family education.
Deliver emotional support and education to patients and caregivers based on individual needs.
Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation.
Respond to urgent needs participate in admissions/on-call and adjust plans based on ongoing monitoring.
Ensure accurate compliant clinical records and high-quality compassionate care delivery.
Qualifications:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN.
