The Department
HARP is a service within the range of Health Independence Programs (HIP) and reports to the Acute and Integrated Care clinical program within the HAC program. HARP aims to prevent avoidable hospital presentations and admissions. The Alfred HARP services achieve this through multidisciplinary and interdisciplinary models of care. Providing services to people with chronic needs and or complex psychosocial backgrounds. The service is designed with a central hub of discipline specific clinicians and spokes of geographically based teams.
We are currently seeking a Physiotherapist with a strong interest in community based chronic and complex holistic health care
The Role
The HARP Physiotherapy role works as part of the HARP interdisciplinary team to provide comprehensive assessment short-term intervention and involvement in maintenance and optimisation of functionality and well-being of clients in the community setting. The role promotes the health and independence of clients and facilitates their ability to continue living safely within the community.
Key Responsibilities
- Undertake and document client assessments treatment and discharge information in EMR in line with departmental and Alfred Health policies.
- Develop and document physiotherapy care plan goals that are person oriented time-limited and measurable; in the SMART format.
- Deliver direct clinical care as part of a HARP respiratory clinical pathway or alternately in collaboration with individualised care teams that address the identified needs and goals of clients with chronic and complex health and psychosocial conditions.
- Provide short term physiotherapy interventions and refer to community based rehabilitation or Primary care services (within or external to the organization)for longer term rehabilitative physiotherapy or ongoing physiotherapy follow up.
- Provide or contribute to comprehensive discharge planning to give clients their best chance of maintaining or improving their health independence and to mitigate avoidable hospital readmission.
- Ensure effective communication and collaboration with HARP team members to deliver a well-coordinated episode of care for clients with whom you work.
- Liaise effectively with referrers and health care providers across acute subacute and community settings to provide timely and accurate advice regarding client PT-related needs.
- Be accountable for individual Key Performance Indicators and use best available data to help inform own practice.
- Provide physiotherapy clinical care within scope of practice using current best evidence
- This role may require work across campuses within Alfred Health (Alfred Sandringham and Caulfield)
Qualifications and Experience Required
Essential
- Registration with the Australian Health Practitioner Regulation Authority (AHPRA)
- Bachelor of Physiotherapy or Equivalent
- Minimum 2 years experience
- Proven clinical experience in an acute hospital setting and/or community setting
- Current Victorian Drivers Licence
Desirable
- Experience working with chronic disease (Cardiac respiratory) including behaviour and lifestyle change interventions health literacy interventions and/or acute/subacute general/geriatric medicine inpatient experience
- Community Physiotherapy experience including home visiting
- Knowledge of respiratory conditions and management desirable
- A demonstrated interest in continued learning opportunities
