Responsibilities:
- Assist residents with ADLs (bathing dressing feeding hygiene)
- Monitor and report changes in patient condition to nursing staff
- Ensure patient safety comfort and overall care support
- Administer medications per QMAP guidelines
- Document medication administration and patient updates accurately
- Support nursing staff with routine clinical tasks
- Maintain cleanliness and infection control standards
- Communicate effectively with healthcare team members
Requirements:
- Active CNA certification required
- QMAP certification required
- Experience in skilled nursing facility preferred
- Basic patient care and medication administration knowledge
- Strong communication and teamwork skills
