Job Title: Registered Nurse - Home Care

Department: Hudson Home Care

Location: ElderONE - Hudson Pace Center

Hours Per Week: 40

Schedule: Monday - Friday 8:00 AM - 4:30 PM

SUMMARY

As a Registered Nurse in Home Care your exceptional ability to teach counsel and empathize with each patient leads to an enhanced quality of life.

RESPONSIBILITIES

• Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;

• Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical emotional and environmental problems

• Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff

REQUIRED QUALIFICATIONS

• Nursing Diploma required

• License and current registration to practice as Registered Professional Nurse in New York State

• Current NYS Drivers license

PREFERRED QUALIFICATIONS

• ASN preferred

• Bachelors degree of Science in Nursing preferred

• 1 year of experience working with a frail or elderly population preferred

• Availability of Motor Vehicle

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
