FUNCTION: Provides indirect and direct clinical support to people with disabilities throughout Achieva Support. Responsible for providing guidance to other clinical team members including other clinical staff as assigned to ensure medical services and supports are provided for people with disabilities within multiple Achieva Support programs. Assesses and reports clinical concerns as they arise. Educates staff and people supported on medical care requirements and health and wellness enhancements. Acts as a liaison to internal and external healthcare professionals to promote health and safety to people supported. Assists with medical appointments as needed. Reports to the Director of Clinical Services.


RESPONSIBILITIES:

  • Supports the achievement of every persons personal outcomes/goals by promoting good health practices and advocating for proper medical and psychiatric care.
  • Adheres to Achieva and Achieva Support policies procedures mission and core values.
  • Provides clinical nursing assessments; guidance and consultation for medical situations involving people supported throughout Achieva Support to ensure their medical and behavioral health needs are addressed and met.
  • Provides initial and on-going training development and constructive feedback as needed to ensure adherence to Achieva policies procedures mission and core values.
  • Communicates external/contracted healthcare practitioner recommendations to applicable Achieva Support employees; ensuring their understanding and adherence to all prescribed medical orders facility discharge instructions and/or health promotions outlined in the persons plan of care and/or health screening tools.
  • Collaborates with program Vice Presidents and/or their management as needed on clinical recommendations made for persons supported; as well as identifying any clinical and mental health needs relating to their respective programs.
  • Provides clinical instruction/training to Achieva Support employees and/or management as needed.
  • Serves as a consultant and contributor for the development of clinical policies and procedures and/or training for the agency.
  • Responds to employees by phone or on-site in the event of a crisis situation and/or medical emergencies ensuring adherence to policies and procedures and that the appropriate parties both professionals and family have been notified.
  • Participates in an on-call rotation attending to calls from employees and/or management that occur after regular business hours.
  • Coordinates hospitalizations admissions/discharges and/or transfers to skilled aftercare facilities and maintains daily contact w/ hospital personnel and/or skilled nursing facility (SNF) staff to obtain clinical updates on persons supported by Achieva. Discuss therapy progress; advise hospital/SNF personnel of what person needs to return safely to their home. Coordinates all discharge needs as applicable.
  • Attends internal and external team meetings for people supported when requested.
  • Serves as a liaison between the pharmacy and Achieva staff. Assists with the obtaining of new medication orders and discontinuing previous medication orders; performs re-orders when requested and medication review when necessary of pharmacy-generated forms.
  • Monitors and participates in state nursing network associations; and tracks state health care initiatives for best practices in the clinical and IDD systems.
  • Participates in any applicable program licensure inspections.
  • Participates in the completion of the HRST process. Serves as a Clinical Reviewer once all required training is completed.
  • Maintains regular contact with the Director of Clinical Services to provide updates regarding current clinical concerns/needs of all Achieva programs; and collaborates with the Director for a plan of resolution of these clinical concerns.

  • Become proficient in the Therap system accurately process all pharmacy orders by converting them into MAR entries and prepare and configure monthly Medication Administration Records (MARs).
  • Assist with the procurement of medical equipment and ensure appropriate training is provided for all devices used by individuals receiving support.
  • Maintain regular communication with the Directors of Clinical Services and Complex Care regarding clinical needs and follow-up plans.
  • Provide education as needed on nutrition hydration special diets skin integrity infection prevention and elimination needs.
  • Assist with developing and implementing person-centered Care Plans for the people in support.


Qualifications
  • Graduate of an accredited nursing program; valid Pennsylvania LPN or RN license (BSN preferred).
  • Minimum 2 years of experience supporting individuals with intellectual disabilities and medically complex needs.
  • Strong organizational communication interpersonal skills and computer proficiency required.
  • Ability to travel locally and work some evenings/weekends including after-hours oncall.
  • Satisfactory Act 33/34 and FBI clearances; reliable vehicle required.
