Independent practice Four-day work week Integrated Behavioral Health Support

Summary

Join CentraCares Northway Clinic as a Family Medicine Advanced Practice Provider (NP or PA) and build your own patient panel while delivering comprehensive care to patients of all ages. This role offers the opportunity to practice independently in a supportive primary care environment managing acute and chronic conditions providing preventive care and coordinating care with integrated behavioral health and specialty support resources.

Schedule

• Full-time 0.77 FTE Onsite CentraCare- Northway Family Medicine St. Cloud MN

• Days 4 8-hour shifts 8:000am 5:00pm

  • Mondays Tuesdays Wednesdays and Fridays

• No nights weekends holidays or call responsibilities

Pay and Benefits

• Salary range:$109241.60 - $124904 per year

  • Pay may be determined by factors such as education experience skills knowledge location and internal equity

  • Salary and salary range are based on a 1.0 FTE reduced FTE will result in a prorated offer rate

  • May be eligible for additional pay through bonuses quality incentives or production-based (RVU) compensation.

Qualifications

Required:

• Graduate of an accredited Nurse Practitioner or Physician Assistant program

• Eligibility for Minnesota licensure as a Nurse Practitioner or Physician Assistant

• Prior experience as NP or PA in Family Medicine Walk-In Clinic Emergency Medicine or Urgent Care
