DescriptionAs a ACT substance Abuse Specialist you will provide skill trainings case management and crisis intervention to participants with co-occurring disorders by using the team approach of Assertive Community Treatment (ACT). You will provide services based on participant strengths and choices and follow evidenced based practices of psychosocial rehabilitation. You will report to the Director of ACT and will work onsite with community travel.
Responsibilities:
- Conduct all phases of psychosocial rehabilitation and outreach (engagement assessing strengths goal planning safety planning evaluating and summarizing progress).
- Coordinate substance abuse assessments
- Create relapse prevention plans such as wellness recovery action plan
- Help identify natural supports and accessing community resources that support recovery.
- Provide substance abuse services tailored to needs and stage of change
- Coordinate care with inpatient substance abuse treatment
- Use evidenced based practices when delivering services to participants as applicable.
- Support participant self-reliance.
- Provide skills training for medication management financial management and for all activities of daily living.
- Demonstrate principles and practices of harm reduction to support clients.
- Provide case management and crisis intervention.
- Collaborate with other staff members community agencies/supports and family members about mutually served participants.
- Share a caseload of approximately 48 adults (residing in Saratoga Warren and Washington Counties) with the ACT team members; responsible for the oversight and completion of their treatment planning clinical documentation charting and all necessary OMH ACT and Agency protocols.
- Work closely with selected clients who are demonstrating an increasing concern with substance use as directed by Director.
- Participate in daily meetings surrounding case reviews crisis intervention clinical formulation treatment planning and additional client/program needs.
- Participate in agency trainings that in turn support our clients needs as assigned by Director
- Must be willing to transport clients in the agency vehicles when applicable/appropriate.
- Provide on call for after hours and weekends with rotation of other team members
- Other tasks as assigned.
Requirements and Education:
- Associates degree and at least one year of directly related experience assisting individuals with substance use and mental illness required.
- A valid NYS drivers license is required as you will provide community-based services and travel is required
- Credentialed Alcoholism and Substance Abuse Counselor license required (CASAC or CASAC-T)
