As a Internal Medicine Physician on our interdisciplinary team youll have the opportunity to form lasting meaningful relationships with your patients and make a tangible difference in their lives every day.

Unlike traditional primary care settings our value-based PACE program empowers you to provide comprehensive integrated care focused on quality not volume. With a smaller patient panel youll be able to dedicate the time and attention each participant deserves.

What Youll Do:

Conduct thorough in-person physical and wellness assessments.
Collaborate with the Interdisciplinary Team to design and implement individualized care plans.
Coordinate participant transitions between acute short-term and long-term care settings.
Focus exclusively on holistic care no hospital rounds with an average of eight patients daily.

Requirements

Completion of an accredited residency program (ACGME required)
MD or DO degree
Board Certification preferred
Active unrestricted California state medical license
State CDS certification and DEA registration
Eligibility to participate in Medicare and Medi-Cal programs
Specialties considered: Internal Medicine Family Medicine and Geriatrics
